The Tuscarawas County Health Department has introduced a convenient online system for ordering birth and death certificates. We encourage you to review the ordering information on this webpage before clicking the button below to access our online ordering platform provided by Permitium.

Birth Records
If someone was born in Ohio in 1908 or later, you can obtain a certified copy of their birth record from us. Starting in 2010, all health departments in Ohio are able to issue birth records for births that took place in Ohio, no matter where the birth happened. Feel free to reach out to us or your local health department for further details. For births that took place outside of Ohio, please refer to the Centers for Disease Control National Center for Health Statistics.
Death Records
If someone passed away in Tuscarawas County in 1908 or later, you can obtain a copy of the death record from us, unless the death took place within the New Philadelphia City limits. In that case, please reach out to the New Philadelphia City Health Department for death certificates. For deaths that happened outside of Tuscarawas County, you should contact the health department of the respective county or city where the death occurred. If the death was outside of Ohio, you can refer to the Centers for Disease Control National Center for Health Statistics.
Prior to 1908
If the birth or death happened before 1908, you can contact the Probate Court of the county where the birth or death occurred.
How do I get a copy of a birth or death certificate from the Tuscarawas County Health Department?
In Person
You’re welcome to visit our office during regular business hours: from 8 am to 4 pm on Monday to Wednesday, 8 am to 6 pm on Thursday, and 8 am to 4 pm on Friday (we're closed on holidays). To ensure we can assist you completely, we kindly ask that you arrive at least thirty minutes before closing time. Certificates are processed while you wait and typically take 15 minutes or less. Please remember to bring the necessary information along with cash or a money order, as we do not accept personal checks. However, credit card payments can be made through our online ordering system, Permitium.

What information do I need to provide?
For a Birth Certificate:
1. Full name of person on birth certificate
2. Date of birth
3. Father's full name
4. Mother's full maiden name
For a Death Certificate:
1. Full name of deceased
2. Date of death
3. If requesting a death certificate for a death that occurred less than 5 years ago, please be able to supply required documentation in order to have the social security number printed on the certificate.
