Birth Records
If the person was born in Ohio in 1908 or later, you can get a certified copy of the birth record from us. Beginning in 2010, all Ohio health departments can issue birth records for Ohio births regardless of where the birth occurred. Check with us or your nearest local health department for more information. For births that occurred outside of Ohio, see Centers for Disease Control National Center for Health Statistics.

Death Records
If the person died in Tuscarawas County in 1908 or later, you can get a copy of the death record from us. If the death occurred outside of Tuscarawas County, you can contact the county or city health department where the death occurred. For deaths that occurred outside of Ohio, see Centers for Disease Control National Center for Health Statistics.

Prior to 1908
If the birth or death happened before 1908, you can contact the Probate Court of the county where the birth or death occurred.

How do I get a copy of a birth or death certificate from the Tuscarawas County Health Department?
In Person
You can stop by our office during normal business hours between 8 am and 4 pm Monday-Wednesday, 8 am and 6 pm on Thursday, and 8 am and 4 pm on Friday (closed on holidays). To allow us to serve you fully, please kindly arrive thirty minutes before closing. Certificates are made while you wait and usually take 15 minutes or less. Please make sure you have the required information and either cash, money order, or a credit card (we welcome payment through Master Card, Visa, and Discover). No personal checks please. 

By Mail or By Phone with Credit Card Payment
To request a certificate by mail, send the following:
  • Completed Application Form and mail to TCHD with a self-addressed stamped return envelope.
  • Check or money order payable to: Tuscarawas County Health Department. Important: We cannot accept personal checks. 
         Please use a money order or cashier's check. You may call us at 330-343-5555, ext. 1250 to provide a credit card payment over the phone.

What information do I need to provide?
For a Birth Certificate:

1. Full name of person on birth certificate
2. Date of birth
3. Father's full name
4. Mother's full maiden name

For a Death Certificate:

1. Full name of deceased
2. Date of death
3. If requesting a death certificate for a death that occurred less than 5 years ago, please be able to supply required documentation in order to have the social security number printed on the certificate.
